Yes, that's a problem. You want your feet to be both pointing street and symmetrical on the bike. If they're uneven you may experience pain and develop muscles unevenly.
The cleat on the left in the picture looks better than the one on the left. Cleats are adjustable and you'll want to adjust them so that the ball of your foot is directly over the center of the pedal and when you're clipped in you have an even amount of side to side play with your heels. You can adjust them with an allen wrench (which was probably included with the shoes and cleats).
Here's a video which has some info on how to adjust the cleats. I would make sure they're even and try a couple different positions until if feels good to you.
